The Company handles the personal information specified in (2) solely within the scope necessary to achieve the following purposes of use. Personal information is not used for any other purpose.
- To enable the access to and management of medical information by the individual or a family member authorized by the individual.
- To enable collaboration with other services related to the Service that the individual wishes to use.
- To share information with medical institutions and related facilities.
- To confirm the consent of the individual regarding the provision of personal information to third parties.
(2) Use of personal information
When operating the Service, the Company handles the following personal information with the consent of the person concerned. The person’s name, date of birth, gender, address, phone number, email address, medical information (hospital visit schedule/history, interview answers, health checkup results, test results such as blood tests and image tests, prescriptions, medical records, etc.), healthcare information (height, weight, blood pressure, number of steps, etc.).
Also, the provided application utilizes the following information on the device. In each case, the information is used assuming the user’s consent has been obtained.
Obtaining location information
Obtain this information to search medical institutions in the user’s vicinity.
Use of camera
Use built-in cameras for the following purposes.
- Taking photos to save in notebooks, vaccine notebooks, talks, and profiles.
- Reading 2D barcodes for data sharing, adding users, etc.
Importing image data
Capturing for display in notes, vaccine notebook, talk, and profile.
(3) Provision of personal information to third parties
The Company shall not provide the personal information determined in (2) to outside parties (third parties) except in the following cases.
- When the user has given consent in advance.
- When it is necessary to protect human life or property.
- When cooperation with public institutions such as police and courts is necessary.
- Other cases based on laws and regulations.
(4) Correction of personal information, suspension of use, etc.
On receiving an application for correction, etc. (correction, addition, deletion), suspension of use (suspension, deletion) of personal information of the individual, the Company confirms the identity of the individual before processing the request. However, this step may be skipped in cases when there is a risk of harming an individual’s life, body, property, or other rights. In case of inability to comply with the request, the Company will explain the reason to the individual.
2. Safety Measures for Personal Information
To ensure the accuracy and safety of personal information, the Company implements organizational, technical, and physical security control measures required by laws and regulations, to prevent unauthorized access to personal information or personal information.
